SCAVENGER CHALLENGE LIABILITY RELEASE FORM

By signing this form, I acknowledge and agree to the following: Voluntary Participation I am voluntarily participating in the CHHA Scavenger Challenge. I understand that participation may involve physical activity and carries a risk of injury. Assumption of Risk I understand and accept that there are risks associated with participating in this event, including but not limited to slips, falls, or other accidental injuries. I accept full responsibility for any and all injuries that may occur as a result of my participation. Waiver of Liability I hereby release and hold harmless the Canadian Healthcare Housekeepers’ Association (CHHA), a Division of the ISSA, its staff, volunteers, affiliates, and event partners from any and all liability for injuries, damages, or losses sustained during the course of the Scavenger Challenge. Medical Acknowledgment I affirm that I am in good physical health and able to participate in this activity. I understand that I am solely responsible for any required medical attention as a result of my participation. Photographs and Media I grant permission to CHHA and ISSA to use photos or video footage taken of me during the event for promotional and educational purposes.
